Step Three: Health Forms

Denver Health Pediatrics at Denver Public Schools: Pediatric E-Consent Form | School-Based Health Centers | Denver Health

All DPS students must provide proof of immunization or a valid waiver. Students who require medication while on campus must also have medication forms completed by their health care provider. These health forms are available on DPS Nursing and Student Health.

Step Four: Free & Reduced Lunch Application

It is important that ALL DMLK families complete the application for Free and Reduced Lunch even if you don’t think you qualify. If you do qualify, you will receive free or reduced lunch as well as discounted fees. Please complete your application HERE.

Step Five: Review FNE Transportation (if needed)

Success Express features a fleet of DPS buses that circulate through the Far Northeast and Central neighborhoods and offer students multiple opportunities to catch a ride. The shuttle runs for several hours in the morning and afternoon, facilitating more flexible school days and on-time access for students to their participating school of choice.

Students can get on or off any bus at any stop. Every bus has two adults on board – the driver and a paraprofessional, whose primary job will be to make sure students are getting on and off at the right spot, and doing so safely.

Click HERE to access the Shuttle Schedules for either the Far Northeast or Central region.

If you have questions or concerns, please email transportation@dpsk12.org or call 720-423-4600.

In addition to the Far Northeast Shuttle, RTD offers free bus and train rides for students. It’s part of their new Zero Fare for Youth pilot program. Click HERE for additional information.
